FY 2005 ANNUAL AND SICK LEAVE RECORD

ID NUMBERS ON HEALTH PLAN ID CARDS
The State of Kansas Division of Personnel Services has advised that all vendors offering services through the State of Kansas Health Plan will be removing Social Security Numbers from the ID cards in the near future. By January 1, 2005, participants will have ID cards with ID numbers that are not SSNs.

The primary reasons for this are to protect privacy and prevent identity theft. However, it will also mean that plan participants should present the new ID cards to their providers as well as ensure that family members residing away from their primary residence receive a copy of the new ID cards. Participants may request additional ID cards by calling the carriers.

Coventry Health Care expects to send out new (replacement) ID cards to all plan participants within the next two weeks. Kansas Choice and Premier Blue began using non-SSN ID numbers on June 1, 2004, for new participants.

The new ID cards will be sent to the employee at their home address, with a cover letter explaining why the cards are being issued. The new ID cards will not change any benefits. All carriers are creating cross-reference files so that historical data such as deductibles, coinsurances, and copays, as well as out-of-pocket and benefit maximums, are transitioned from the old ID card number to the new one.

THELCO PRECISION TABLE-TOP INCUBATOR AVAILABLE
The Diagnostic Laboratory has a Thelco Precision Table-top incubator available to a good home.  This is an older model but it is still in working condition.  If interested, please contact Joy Pierzynski at 532-1383 or Diana Pavlisko at 532-1380.  --Diana Pavlisko

Order Forms Coming
Order forms for 4th Quarter 2004 and September new publications and revisions will be sent June 11. Please remember to place your orders by the deadline, July 1.

4-H Horse Show Rule Book Revised
The 4-H Horse Show Rule Book (S-133) has been revised, and samples will soon be distributed to county offices. Previous orders from the August 2004 order form will be filled and delivered. It is also available online at: www.oznet.ksu.edu/library/4H_y2/samplers/s133.asp .

Publication Updates
MF-2642, Western Corn Rootworm: Kansas Area-wide Management Summary, will be grant funded. Orders received so far, will be filled at no charge. Additional copies are available, but quantities may be limited.
