Update of the Accident/Incident Reporting Process
All campus and K-State activity-related incidents/injuries/illness/accidents should be reported through the Incident Reporting System by the end of the day that the incident occurred (this is an EHS requirement).
K-State employees are required to report a work-related incident or injury within three days if injured while on duty. Agents report to their regional director's office or the Extension Operations Office. Area and campus-based employees report to their supervisor or the department head's office.
All incidents, accidents or injuries are reported through the Electronic Incident Report. It ensures work-related incidents are reported in a timely manner to the State Self Insurance Fund for assessment. Information gathered in the incident reporting process will also be used by the K-State Department of Environmental Health and Safety to support uninterrupted research and learning within a safe and healthy environment at K-State.
A few reminders for work-related incidents:
• If a life-threatening emergency, call 911 immediately and take action to seek the emergency treatment at the nearest emergency facility.
• Urgent (not life threatening but same day treatment needed) and non-emergency treatment must be pre-authorized by the State Self Insurance Fund. Injured employees, Supervisors, or Human Resource Liaisons can call the State Self Insurance Fund at 785-296-2364 to request authorization and follow their treatment plan.
• Supervisors will review and confirm incidents that are reported by employees.
• All medical bills should be submitted directly by the provider to the SSIF at the following address: CompAlliance/SSIF, PO Box 1697, Topeka, KS 66601-1697.
• DO NOT send medical bills to Human Resources, this will only delay the payment.
• The employee should notify his or her medical provider that the claim should be submitted as workers' compensation to the above address. Questions about medical payments should be directed to the SSIF at 785-296-2364.

If office professionals or program assistants of local extension units are injured, contact the local provider of Workers' Compensation insurance for their process of reporting claims and then file an electronic incident report at the link above.
