Extension Board and PDC Demographic Data

The Extension Council Executive Board Members & County Extension Council Members (Form 8-4) or Governing Board of the Extension District (Form 9-4) should be completed and returned to your regional office professional as soon as you have all of the information necessary to complete the form. Both forms are available on the KSRE Forms page. Local units are required to ask board and PDC members to self-report their demographic data. This can be done using the Extension Participant Demographic Data Collection Form.
Start collecting this data now so that it will be available for you to complete the 8-4 or 9-4 form in a timely manner.
